Job DescriptionJob DescriptionSalary: We are looking to bring on aHome Health Clinical Managerexperienced in overseeing and coordinating the clinical aspects of home health care provided in patients' homes. This role ensures high-quality patient care, compliance with regulatory requirements, and efficient use of resources. (PTO policy rewards longevity with increases each year)Competitively priced health plan, vision and dental, partially employer paid!Company paid short and long-term disability and life insurance (up to $50,000 in coverage)401K with employer matchCareer advancement opportunitiesProactive Health Management Wellness Program (PHMP)We Pay for CPR Recertification!Brief Overview of Responsibilities for Home Health Clinical Manager:Supervise field clinicians and all relevant activities pertaining to episodic management and clinical outcomes, and provide progressive discipline as requiredReceive case referrals and assess patients' needs to assign appropriate cliniciansEnsures accuracy with staff to comply with company policy and proceduresProvide staff development opportunities to include in-services and educational classesReview and investigate the clinical content, diagnoses, medical history, medications, fee status and other procedures from the records of clients.Appraise staff performance during their probationary period and annually with the Director of Nursing/Administrator, provide the teaching and counseling necessary to enhance the delivery of services and the professional development of nursing and health aide staff.Identify and evaluate client needs and appropriateness of home care by reviewing the initial intake screening form before assignment of direct care staff.Review each nurse's schedule for recertification, discharges, and attendant supervisory visits.May be requested by the Director of Nursing/Administrator to fill in for field staff when necessary.Manage staff by reviewing weekly reports of productivity, length of stay, hospitalizations, outcomes, and timeliness of care.Other duties as assigned.Qualifications:Preferably two years of home health care experienceAt least 1 year of management/leadership experience, preferably in home healthCurrent Virginia State RN LicenseIf you are interested and are a caring, compassionate nurse who can make a difference in the lives of our patients, then we would be interested in meeting you!!"James River is an equal opportunity employer.
