Job Description: Memory Care Nurse (RN / LPN)
Position Summary
The Memory Care Nurse (RN or LPN) is responsible for providing direct nursing care to residents with Alzheimer’s disease, dementia, and other cognitive impairments. This role ensures the delivery of compassionate, person-centered care while promoting residents’ dignity, safety, and quality of life. The nurse collaborates with interdisciplinary team members to support clinical outcomes and regulatory compliance in a long-term care environment.
Key Responsibilities
Resident Care
- Deliver skilled nursing care in accordance with individualized care plans
- Monitor residents’ physical and cognitive conditions and report changes promptly
- Administer medications and treatments per physician orders and facility policy
- Provide dementia-focused care using appropriate communication and behavioral interventions
- Assist with activities of daily living (ADLs) as needed
Clinical Oversight
- Conduct assessments, including admission, quarterly, and change-in-condition assessments
- Document care accurately in the electronic medical record system
- Participate in care plan development and interdisciplinary meetings
- Ensure compliance with infection control and safety protocols
Behavioral & Memory Care Support
- Manage behavioral symptoms related to dementia using non-pharmacological interventions
- Support a calm, structured, and therapeutic environment
- Respond appropriately to agitation, wandering, and other dementia-related behaviors
Team Collaboration
- Supervise and support Certified Nursing Assistants (CNAs) or caregivers
- Provide guidance, coaching, and training on dementia care best practices
- Communicate effectively with families, physicians, and care team members
Compliance & Quality Assurance
- Adhere to all state, federal, and facility regulations
- Participate in quality improvement initiatives and audits
- Maintain resident confidentiality and HIPAA compliance
Qualifications
Required
- Current, active RN or LPN license in the state of Indiana (or applicable state)
- Experience in long-term care, assisted living, or memory care preferred
- Strong knowledge of dementia and Alzheimer’s disease care
Skills & Competencies
- Compassionate, patient-centered approach to care
- Strong communication and interpersonal skills
- Ability to manage challenging behaviors with empathy and professionalism
- Excellent time management and organizational skills
- Leadership ability (especially for RN role)
Preferred Qualifications
- Prior experience in a memory care or dementia unit
- Training in dementia care (e.g., CPI, dementia certification)
- Experience with electronic medical records
Work Environment
- Fast-paced long-term care setting
- Interaction with residents experiencing cognitive impairments
- Requires standing, walking, bending, and assisting residents with mobility
RN-Specific Responsibilities (if applicable)
- Lead clinical decision-making and oversee nursing staff
- Perform comprehensive assessments and clinical evaluations
- Coordinate with physicians regarding changes in resident condition
LPN-Specific Responsibilities (if applicable)
- Provide direct nursing care under RN or physician supervision
- Administer medications and treatments
- Report clinical changes and assist with care planning
