Job Description
We are seeking a compassionate and experienced Registered Nurse to serve as a Case Manager within a hospice care team. This role focuses on delivering patient-centered care to individuals with life-limiting illnesses, coordinating comprehensive nursing services, managing symptoms, and providing support to patients and their families throughout the end-of-life process. The RN Case Manager acts as the primary point of contact for patients’ care plans, collaborating with an interdisciplinary team to ensure holistic care addressing physical, emotional, and spiritual needs.Key Responsibilities:Patient Assessment and Care PlanningConduct thorough assessments of patients’ physical, emotional, and psychosocial needs.Develop, implement, and update individualized care plans in collaboration with the hospice team.Monitor and evaluate patient conditions, adjusting care plans to manage symptoms and maintain comfort.Symptom ManagementAdminister medications and treatments to alleviate pain and manage symptoms such as nausea, anxiety, or respiratory distress.Educate patients and families on symptom management techniques and safe medication use.Respond promptly to changes in patient condition, coordinating with physicians and team members for interventions.Coordination of CareServe as the primary liaison between patients, families, and the hospice team, including nursing staff, social workers, spiritual care providers, and aides.Coordinate scheduling of visits and services to ensure timely, patient-focused care.Communicate effectively with all stakeholders to ensure continuity of care and alignment with patient goals and preferences.Patient and Family SupportProvide emotional support and counseling to patients and families, addressing concerns and end-of-life planning.Educate families on caregiving techniques, the dying process, and available hospice resources.Assist with advance care planning, including directives and related arrangements, as needed.Documentation and ComplianceMaintain accurate and timely documentation of patient assessments, care plans, interventions, and communications according to regulatory standards and organizational policies.Ensure compliance with applicable healthcare regulations and participate in quality assurance and performance improvement activities.Interdisciplinary CollaborationParticipate in interdisciplinary team meetings to review patient progress and update care plans.Collaborate with hospice aides, volunteers, and other team members to provide comprehensive care.Advocate for patients’ preferences and needs within the care team.Qualifications:Education and LicensureAssociate or Bachelor’s degree in Nursing (BSN preferred).Current, unrestricted Registered Nurse (RN) license in the state of practice.Certification in Hospice and Palliative Nursing (CHPN) preferred or willingness to obtain within a specified timeframe.ExperienceMinimum of 1–2 years of nursing experience, preferably in hospice, palliative care, or related fields such as oncology or geriatrics.Experience in case management or care coordination is an advantage.Skills and AbilitiesStrong clinical assessment, critical thinking, and decision-making skills.Excellent communication and interpersonal skills to build trust with patients, families, and the care team.Ability to work independently and manage a caseload efficiently with strong organizational skills.Compassionate, empathetic, and culturally sensitive approach to patient care.Proficiency in electronic medical record (EMR) systems and accurate documentation practices.About the Role:This position provides an opportunity to deliver meaningful, patient-centered hospice care while coordinating with an interdisciplinary team to ensure holistic support for patients and their families.
