About the PositionThe Population Health RN Risk Adjustment Specialist will play a critical role in improving clinical documentation for Hierarchical Condition Category (HCC) coding accuracy across a portfolio of primary care practices. This position bridges clinical expertise with health technology to ensure documentation supports accurate risk adjustment quality reporting and optimal patient care outcomes.About Outpatient ClinicsCorewell Health operates over 300 outpatient clinics across Michigan bringing a multitude of care options to the communities that we serve. There are over 40 different types of primary care and specialty practices which allows those who work in these clinics the opportunity to learn and explore various interests or to specialize in one area.Scope of workServes as a population health subject mater expert in the practice. Plans and prioritizes care for individuals and population of patients focusing on strategies that will promote optimal health within populations and directs patients to the right care team member at the right expertise current knowledge in nursing care and management of a sub population of patients of varying complexity and seeks to improve patient family and health systems / community outcomes through the application of educational concepts / skills and preventive care in a managed care environment.QualificationsBachelors Degree or equivalent in nursing or a healthcare related field Required3 years of relevant experience clinical nursing and or case management/managed care or related field RequiredLIC-Registered Nurse (RN) State of Michigan Upon Hire RequiredCRT-Basic Life Support (BLS) - AHA American Heart Association Ambulatory Upon Hire Required or CRT-Basic Life Support (BLS) - ARC American Red Cross Ambulatory Upon Hire RequiredExperience in Primary Care or Population Health PreferredFamiliarity with EHR systems and health technology platforms PreferredProficiency in Microsoft Excel (pivot tables charts data analysis) PreferredStrong presentation and communication skills PreferredDemonstrated ability to manage multiple practices and priorities PreferredKnowledge of HCC coding risk adjustment and value-based care model PreferredChange management experience PreferredHow Corewell Health cares for youComprehensive benefits package to meet your financial health and work/life balance goals.
