Job Title: RN Supervisor – Home Care (TBI & Dementia Program)

Location: Yonkers, NY

Shift/Schedule: Monday – Friday | 9:00 AM – 5:00 PM

Compensation: $90,000 – $100,000+ annually (flexible for highly qualified candidates)


Position Overview

A well-established licensed home care agency is seeking an experienced Registered Nurse (RN) Supervisor to oversee Home Health Aides and coordinate care for individuals enrolled in Traumatic Brain Injury (TBI) and Dementia programs.

This role combines clinical oversight, staff supervision, patient assessments, compliance monitoring, and extensive documentation related to waiver program requirements. The ideal candidate is organized, detail-oriented, and experienced in home care operations.


Key Responsibilities

  • Supervise and support Home Health Aides (HHAs) in the field
  • Conduct patient assessments and develop individualized care plans
  • Ensure compliance with state regulations and waiver program requirements
  • Complete and review clinical documentation and required paperwork
  • Participate in caregiver orientation, training, and competency evaluations
  • Monitor quality of care and address clinical concerns as needed
  • Coordinate with interdisciplinary teams, patients, and family members
  • Perform supervisory visits and maintain accurate records
  • Ensure timely completion of all clinical and administrative documentation


Qualifications

Required

  • Active New York Registered Nurse (RN) license
  • Home care or home health experience
  • Experience supervising caregivers or clinical staff
  • Strong documentation and compliance skills
  • Excellent communication and organizational abilities
  • Valid driver's license and reliable transportation


Preferred

  • Experience with TBI, Dementia, NHTD, or Waiver Programs
  • Case Management experience
  • UAS assessment experience
  • Knowledge of New York home care regulations
