RN System Director of Case ManagementGainesville GeorgiaThe Director of Case Management leads and shapes the Case Management and Social Work services. As the Director of Case Management youll guide discharge planning Care Coordination and access management to ensure patients receive seamless support throughout the Continuum of Care. Additionally the Director oversees pre-certification for post-acute services optimize resource use and promote ICD-10 awareness.
