RN Case Manager Senior Care Options (SCO) Schedule: Monday Friday 8:00 AM 4:00 PM Work Style: Hybrid 2 days remote 3 days in the field or office No nights no weekends no on-call! Are you a compassionate and experienced Registered Nurse ready for a role with better balance and bigger impact Were seeking an RN Case Manager to join our Senior Care Options (SCO) team where youll help older adults with complex needs navigate their health journeys and remain safely at home. This is more than a typical nursing role. As a case manager youll build long-term relationships with members and families create individualized care plans and coordinate services to improve health outcomes. And with a consistent weekday schedule and a hybrid work setup youll have the flexibility and support you need to thrive.
