Position Summary

The Licensed Practical Nurse (LPN) is a key clinical support provider responsible for ensuring residents receive high-quality care in Assisted Living and Memory Care.

This role supports assessments, medication management, care plan implementation, and ongoing monitoring of resident wellbeing while working closely with the interdisciplinary care team.

Key Responsibilities

  • Administer medications and treatments per physician orders

  • Assist with resident assessments and care planning

  • Monitor and report changes in resident condition

  • Maintain accurate clinical documentation

  • Support caregivers and medication aides

  • Ensure care is delivered with dignity and respect

  • Collaborate with physicians, families, and leadership

Qualifications

Required

  • Active LPN license (Tennessee)

  • Ability to work full-time 2nd shift

  • Strong clinical and communication skills

  • Reliable attendance and teamwork mindset

  • Passion for working with seniors

Preferred

  • Assisted Living, Memory Care, or long-term care experience

  • Previous senior living experience
