What Youll DoHit the road (locally!) to visit clients in their homes.Be the go-to guide for families navigating waiver programs.Write up plans of care and assessments that actually make a difference.Troubleshoot challenges like service authorizations, timesheets, and more.Build lasting relationships with families built on respect, patience, and trust.Work remotely from your laptop/tablet. What Were Looking For At least 2 years of experience supporting individuals with disabilities or the elderly. No degree required (we care more about heart and experience). Tech-savvy enough for email, docs, spreadsheets, portal navigation and video calls. A valid drivers license & reliable vehicle (no client transport). Great communicationboth written and spoken. Able to pass a background check + provide 2 professional references. Bonus points if youve got Person-Centered Thinking/Planning training.
