Job Description

Every other weekend: Saturday & Sunday. 8 hours a day- 16 hours a pay period. About Department: This position provides support for behavioral health patients in the HDVCH Emergency Department Inpatient Pediatrics and Med/Psych Unit. This social worker provides assessment care coordination recommendations therapeutic interventions and safe discharge planning for children admitted to HDVCH with acute behavioral health needs.Scope of Work: Utilizes clinical judgment and expertise to coordinate care management/behavioral health interventions. Develops strategies to ensure cost effective quality care outcomes to patients with complex care needs. Identifies options for levels of care for individuals and populations of patients focusing on strategies that will promote optimal health. Assesses plans monitors and provides therapeutic interventions as appropriate. Collaborates with patient and family to facilitate understanding of diagnosis and to provide therapeutic interventions to support necessary lifestyle and behavior changes. Partner with patients families and other health care providers to ensure understanding the impact the disease or illness has on the patients mental and emotional health. Respond to crisis situations and engage with patients and families to access the appropriate level of care across the continuum. Emphasis of duties may vary depending upon area of assignment.Works in close collaboration and communication with physicians and other members of the health care team in order to minimize duplication of effort promote teamwork and ensure seamless coordination of services within and between teams and across campuses.Assists with patient evaluation and treatment to further their understanding of significant social and emotional factors underlying a patients health problem(s). Actively participates in care conferences as appropriate.Mobilizes patient/significant others to utilize their own strength to make necessary lifestyle and behavior changes. Acts as a patient advocate to ensure their desires are recognized and followed to the greatest extent possible.
