DescriptionJob Title: Social Worker - Heart & Kidney Transplant (Part-Time)Social Work Services at the Mount Sinai Hospital The Mount Sinai Hospital is a 1134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. We believe that total patient care must emphasize the physical emotional and social needs of each patient and their care partners. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907 the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education professional development and extra-curricular opportunities.The social worker in this position will join three Cardiac Transplant/VAD (Ventricular Assist Device) social workers and will be an integral member of the team.ResponsibilitiesThe social worker will be responsible for following cardiac transplantVAD and TAH (Total Artificial Heart) patients which involves assessing patients suitability for transplantation/VAD TAH following patients while hospitalized as well as out- patients both pre and post-transplant/LVAD/TAH as needed. This is a complex position that requires juggling the clinic and in-patient work. This social worker will also be a part of medical interdisciplinary team to coordinate psychosocial components of patient care. The social worker will have responsibilities that include leading support groups teamwork committee meetings quality improvement and other integrative services to benefit the patient population and a weekly 8:00 AM recipient review meetings. There is a strong focus on program development and patient satisfaction. The worker will be a part of the Heart/VAD and Lung Transplant Coverage Group.This is a part-time position (3d/week) involving a Job Share with another Social Worker. The person in this role would work in Heart Transplant 2 days/week. The third day (which must be Friday due to the structure of the job share) requires working in Liver/Kidney transplant as part of a job share arrangement in that area. The roles and responsibilities in Liver/Kidney transplant are very similar to those in Heart Transplant but would require collaboration with those teams one day/week. The person in this part-time role should have excellent collaboration and documentation skills as well as a track record of agility and ability to transition seamlessly between units/services.QualificationsNYS LMSW or LCSW. In-patient Cardiac experience preferred.Excellent clinical group work and organizational skills.Ability to assess needs and coordinate resources within hospital and community networks for effective management of patient care and expedious safe discharge planning. Experience in providing individual and group psychosocial services to patients with acute and chronic illness.Exceptional collaborative and communication skills with multiple staff and units as well as great flexibility.Experience with anticipatory grief and end-of- life counseling. Excellent presentation skills.Ability to develop programs.LCSW preferred.Minimum one year in current position.
