The Transplant Program Social Worker provides coverage for recipient pre-transplant evaluations for the Heart Transplant Program as well as assists with coverage for post-transplant outpatients The Social Worker is responsible to providing transplant patients a specified social work assessment and clearance prior to transplant. This position requires the Social Worker to be competent in the transplant process including specific psychosocial assessment for transplant patients and the ability to educate and guide patients through post-transplant recovery and any transplant complications. The post-transplant regimen is very rigorous and the Social Worker needs to be knowledgeable and needs specific education and preparation to ensure that the SW must ensure has occurred. The transplant SW is an integral part of the interdisciplinary team and works closely with all members to ensure that patients are cleared for transplant and have a safe and stable post-transplant plan for a successful return to the community This is a Monday - Friday 9am-5pm 1199 Union position Preferred Criteria: Bilingual Spanish At least one year of inpatient Medical/Surgical experience Required Criteria: Master’s Degree in Social Work (MSW) New York State License as a Licensed Masters Social Worker (LMSW) Join a healthcare system where employee engagement is at an all-time high.
