Belmont Bay Rehabilitation and Healthcare Center - Join our team at Belmont Bay Rehabilitation & Healthcare Center as a Social Worker.Proudly supported by Marquis Health Consulting Services. Responsibilities of a Social Worker: Evaluate patients (existing, new, and prospective) based on their needs and limitations.Address patient concerns and goals while maintaining constant communication with the patient, as well as the case manager.Build rapport with patients and their families. Coordinate care with PT departments. Develop care plans for patients. Qualifications of a Social Worker: Bachelor’s degree required. A master’s degree is strongly preferred.Experience as a social worker in a healthcare-related setting.
