Social Worker Per Diem at University of Wisconsin Hospitals and Clinics Authority

DescriptionWork Schedule:This is a per diem position. Schedule will vary and be flexible however most shifts fall between the hours of Monday Friday 8:00 am 4:30 pm.Be part of something remarkableThis is a new unique opportunity to work with both adult and pediatric patients in a variety of areas including inpatient work at University Hospital American Family Childrens Hospital and East Madison Hospital as well as within the Emergency Departments at University Hospital and East Madison Hospital.We are seeking a Per Diem Social Worker to:Evaluate complex patient and family situations which have legal ethical and psychosocial components.Collaboratively develop patient goals monitor progress address barriers to goal achievement and evaluate outcomes of individualized care plans.Consider barriers including coping with chronic illness mental health history/symptoms cognitive development adverse developmental events resistance to treatment health system impediments lack of social support and vulnerability.Participate in discharge planning for patients requiring post-discharge services which may include transitions to skilled nursing facilities assisted living facilities outpatient dialysis Acute Rehab hospice and transportation needs.Recommend consultation requests as appropriate.Engage patient/family in the treatment and decision-making process with respect to patient rights principles of confidentiality respect for patient privacy and right to self-determination.Utilize crisis intervention conflict resolution and mediation to address patient/family concerns.At UW Health you will have:Options for a variety of schedules and shifts that offer flexibility and allow for work-life balance.Access to great resources through the UW Health Employee Wellbeing Department that supports your emotional financial and physical well-being.Tuition benefits eligibility - UW Health invests in your professional growth by helping pay for coursework associated with career advancement.The opportunity to earn a referral bonus for referring friends former colleagues or others to apply for open posted positions.QualificationsMasters Degree in Social Work RequiredWork Experience2 years prior hospital based social work experience Preferred2 years prior hospital or community-based social work experience if supporting the HIV Clinic Preferred2 years prior hospital or clinic based pediatric experience if supporting American Family Childrens Hospital (AFCH) PreferredLicenses & CertificationsCertified Advanced Practice Social Worker (CAPSW) in the state of Wisconsin within 90 Days Required orCertified Independent Social Worker (CISW) in the state of Wisconsin within 90 Days Required orLicensed Clinical Social Worker (LCSW) in the state of Wisconsin within 90 Days RequiredOur Commitment to Social Impact and BelongingUW Health is committed to fostering a workplace that creates belonging for everyone and is an Equal Employment Opportunity (EEO) employer.
