Job Title: Social Worker Location: Manistique MI Schedule: Full-time (5 8s) - Open to 4 10s Compensation: $27 $37/hr (negotiable) $5000 Sign-On Bonus (potentially negotiable) Certification: LSW or LLSW preferred Education: Masters Degree in Social Work required Job Description: The Social Worker provides compassionate comprehensive social work services to patients and families within both the HomeCare and Hospice programs. This role focuses on assessing and addressing the emotional social and psychosocial needs associated with illness treatment and end-of-life care. The Social Worker collaborates as an integral member of the interdisciplinary care team to ensure high-quality patient support and seamless coordination of services. Key Responsibilities: Conduct psychosocial assessments to identify patient and family needs related to illness coping and support. Provide counseling emotional support and crisis intervention to patients and families. Participate actively in interdisciplinary team meetings to assist with treatment planning and care coordination. Manage and assist with discharge planning community resource referrals and advanced care planning. Guide patients and families in completing advanced medical directives and understanding available care options. Perform required assessments and screening tools in alignment with program standards. Provide supervision to bachelor-level social workers and ensure adherence to best practices. Maintain accurate timely documentation in compliance with organizational policies and regulatory guidelines. Support both HomeCare and Hospice clinical teams to optimize patient experience and continuity of care. Preferred Qualifications: Licensed Social Worker (LSW) or Licensed Limited Social Worker (LLSW). Experience in HomeCare Hospice or medical social work (preferred but not required). Strong organizational skills empathy communication and ability to work independently and collaboratively.
