*** This position is a stipended position and is only open to current employees whose current job status allows them to participate in these opportunities.***Role Overview:In partnership with the Capital Planning Senior Project Manager the School-based Transition Coordinator will work closely with the school leadership team at the Frederick or the West Zone School to develop and maintain a system to support students and families impacted by a school transition due to a closure merger or consolidation. As a dedicated team member who knows the school community well the Coordinator will serve as the primary on-site point of contact for students and families experiencing a transition and will be an integral part of ensuring the best school transition experience possible.Transition Operations SupportProvide one-on-one transition advisory support and case management to students impacted by a school closure merger or consolidationCollaborate with other student support services teams (Social Workers Family Liaisons and Welcome Services team) on student and family transition supportsMaintain student transition files including documented plansWorks with school leader to develop and operationalize packing and moves including coordinating staff breaks for packing coverage and timeline working within the appropriate union contractsCommunity Engagement and PartnershipsServe as the primary on-site point of contact for students and families working closely with the Capital Planning Senior Project Manager to address questions and concernsCoordinate resources and supports for families and students experiencing hardship/challenges due to school closuresAs a member of the school transition planning team work with the Capital Planning Senior Project Manager to help plan and implement transition events for students and familiesIn partnership with the Capital Planning Senior Project Manager schedule coordinate and attend community meetingsDevelop information sharing systems across multiple mediums (website telephone blasts backpacked materials etc.)Meet monthly with the Capital Planning Senior Project Manager in support of planning effortsMust be available evenings and weekends to support community engagement effortsRequired Qualifications:Current employee at impacted schoolDemonstrated commitment to diversity equity and inclusionStrong relationship-building skillsStrong verbal and written communication skillsAbility to listen and promote empathyProject and/or program management experienceExcellent organizational skillsStrong team-building skillsDecision-making skillsComputer literate proficiencyinternet GoogleSuite Microsoft OfficeTerms: $56.81/hrRequired Experience:IC Key Skills Experience Working With Students,Organizational skills,Classroom Management,Behavioral Health,Classroom Experience,Mediation Experience,Special Education,Conflict Management,Teaching,Experience with Children,Public Speaking,Word Processing Employment Type : Grant Experience: years Vacancy: 1
