**Join Our Compassionate Case Management Team ****2025 VT Best Places toWork**Are you a dedicated and caring individual with a passion for making a positive impact on the lives of older adults and/or people with disabilitiesIf so we have the perfect opportunity for you to join our growing and compassionate case management team in Windsor VT!At Senior Solutions (Area Agency on Aging in Southeastern Vermont) we are seeking aCase Managerto work with older adults and/or people with disabilities inWindsorVermont and the surrounding areasusing a person-centered planning approach and a deep understanding of state and federal benefits. Strong organizational communication and computer skills are required.Qualifications:Bachelors degree in human services and prior case management experience preferred.A combination of relevant education and experience may be substituted for a degree.If you have a passion for working with seniors or adults with disabilities then you will fit right in! We are always looking for team members who are driven to quickly learn new skills about all aspects of the benefits and services available to aging Vermonters.
