Job DescriptionJob DescriptionSalary: EPIC Health Partners is seeking a dedicated and compassionate Substance Use Case Manager to support individuals and their families in accessing essential services related to substance use and co-occurring disorders. This role is vital in helping clients achieve recovery and stability through person-centered, culturally appropriate care coordination and advocacy.Experience and Professional Qualifications:Required:Bachelors degree in psychology, social work, counseling, vocational rehabilitation, or other VA DMAS-approved Human Services field; or certification as a Psychiatric Rehabilitation Practitioner in VA.Preferred:Masters degree in psychology, social work, or counseling.Experience:At least one year of experience in substance use or mental health services within the past seven years preferred.Staff must be currently certified in: CPR, First Aid, and have verification of Tuberculosis screening.Key Responsibilities:Assess needs and plan services, to include developing a substance use case management individual service plan (ISP)Enhance community integration opportunities and link individuals to community supportsAssist the individual directly to locate, develop, or obtain needed services, resources, appropriate public benefits and transportation as neededAssure the coordination of services and service planning within a provider agency, with other providers, and with other human service agencies and systems, such as local health and social services departmentsMonitor service delivery through contacts with individuals receiving services, service providers, and site and home visits to assess the quality of careAdvocate for individuals in response to their changing needs, based on changes in the ISP.Act as the primary point of contact for individuals and the Interdisciplinary Care Team (ICT), lead ICT process and meetings, facilitate communications among relevant parties regarding individuals attendance, drug test results, etc.Assess for appropriate changes or additions to services, facilitates referrals for the members, and ensures the ISP is updated as necessaryAttend and participate in required meetingsComplete all required/accompanying documentationMaintains confidentiality and security of Protected Health Information (PHI)Complete additional duties as assignedKnowledge, Skills, and Abilities:Strong understanding of substance use and co-occurring disorders, and ASAM criteriaFamiliarity with psychotropic medications and their side effects.Skilled in service planning, care coordination, and documentation.Excellent communication, problem-solving, and time management skills.Ability to work independently and collaboratively in a team setting.Knowledge of community resources, crisis intervention, and confidentiality laws.Experience and Professional Qualifications:Required:Bachelors degree in psychology, social work, counseling, vocational rehabilitation, or other VA DMAS-approved Human Services field; or certification as a Psychiatric Rehabilitation Practitioner in VA.Preferred:Masters degree in psychology, social work, or counseling.Experience:At least one year of experience in substance use or mental health services within the past seven years preferred.Staff must be currently certified in: CPR, First Aid, and have verification of Tuberculosis screening.
