Success Coach for El Puente ProgramFamilies & Youth Innovations Plus (FYI)Position: Full-TimeHourly Rate: $17.98 - $21.76 (3 levels based on experience and educationFLSA Status: Hourly/Non-ExemptReports To: LVP Program SupervisorDepartment: La Vida Project - El Puente ProgramJob SummaryThe Success Coach provides specialized case management services to individual client needs. They also create programs to benefit clients and educate clients on available community resources. Success coaching ensures that clients receive LVP services in a timely appropriate efficient and coordinated fashion.What youll Do:Key ResponsibilitiesProvide clients with supportive servicesCoordinate services between providers and clientsInterpret agency services to clients and communityProvide case management services and skill-building services to youth in the programProvide housing case management servicesMaintain individual case records on clients receiving servicesProvide specialized case management services to individual client needsEducate clients on available community resources and servicesCreate programs and services to benefit clients and the communityAttend community meetings relevant to program and clientsAdminister and facilitate the completion of appropriate intake forms assessments intake packet andservice plansConduct field visits attend meetings and be available to deliver services during traditional and nontraditionalhours (when needed) in; the clients home educational institutions places of employment FYIs Main office JPPO offices detention facilities reintegration centers or any other setting that enables and promotes the consistent and structured delivery of LVP servicesOrganize and facilitate Life Skills Groups as needed in both individual and group settingsRequired to participate and interact in youth activities (for example: camping sport activities overnight youth conferences etc.) which may involve occasional overnight activities and tripsWhat You Need:Qualifications and ExperienceBachelors degree in Social Work Family and Consumer Sciences Criminal Justice PsychologySociology or Criminology from an accredited College or University.Must pass a CYFD Criminal Records CheckMust maintain a full certification of the following trainings; CPR/First Aid and CPIMust maintain a valid NM Drivers Licenses.
