We are currently seeking a part-time SUD Discharge Planner Registered Nurse (RN) to join our teamat MarylandCorrectional Institutelocated in HagerstownMaryland.The SUDDischarge Planner RN completes discharge planning for patients referred to the multidisciplinary team based on the determination that community resources will be required at the time of release. This person will accomplish timely and appropriate discharge plans act as a liaison between correctional healthcare teams and community agencies.
