We need individuals who are passionate about making a difference by helping others.We are seeking a Supports Coordinator.This role isresponsible for Person-Centered planning to develop an Individual Plan of Service. The Supports Coordinator will provide on-going linking coordinating and monitoring of services.This is a full-time hybrid position working Monday to Friday from 8am to 5pm. While the position is remote this role will meet with clients in the community as part of their responsibilities.MINIMUM QUALIFICATIONS:Bachelors degree in human services field AND one year of experience treating or working with a person who has intellectual disability or specialized training received through fieldwork and/or internships.Possession of a valid drivers license and access to reliable transportation for job related useStrong organizational skills with experience in person-centered planningKnowledge of Culture of GentlenessDemonstrated understanding of persons with developmental disabilitiesDemonstrated communications skills that effectively convey informationDemonstrated active listening i.e. listening to what other people say and asking appropriate questionsDemonstrated time management skillsLived experiences with mental illness/developmental disabilities/substance use disorders valuedPREFERRED QUALIFICATIONS:Licensed BSWKnowledge of trauma-informed theories principles and practices (includes multi-faceted understanding of concepts such as community trauma intergenerational and historical trauma parallel processes and universal precautions).Job interviews are being conducted in person or via the perform this job successfully an individual must be able to perform each essential duty satisfactorily.
