JOB DESCRIPTION

The Staff Pad has partnered with Blue Ridge Medical Center to hire a Swing Bed Coordinator to support care transitions, patient flow, and program growth within a critical access hospital setting.

This role sits at the intersection of case management, discharge planning, and program coordination , ensuring seamless transitions for patients entering and exiting swing bed services while optimizing resource utilization and patient outcomes.

Position Overview

The Swing Bed Coordinator is responsible for managing the day-to-day operations of the swing bed program, serving as a key liaison between clinical teams, patients, families, and external providers. This individual will play a critical role in ensuring compliance, improving patient experience, and supporting the growth and efficiency of the program.

Key Responsibilities
  • Coordinate all aspects of swing bed admissions, discharges, and transfers to ensure smooth and timely patient transitions
  • Serve as the primary point of contact for patients, families, clinical staff, and referral sources
  • Support discharge planning efforts and collaborate with care teams to ensure appropriate patient placement and follow-up care
  • Maintain accurate documentation and patient records in compliance with regulatory requirements
  • Manage scheduling and resource coordination within the swing bed unit
  • Assist with program coordination efforts, including communication with internal and external stakeholders
  • Provide administrative support, including phone coverage, front desk assistance, and general office coordination
Qualifications
  • RN, LPN, Case Manager, or Social Worker background preferred
  • Experience with Medicare swing bed regulations strongly preferred
  • Background in discharge planning and care coordination
  • Understanding of rural hospital operations a plus
  • Strong communication, organizational, and relationship-building skills
  • Familiarity with therapy and rehabilitation services preferred
  • Proficiency with Microsoft Office and general documentation systems
