DescriptionLocation:Newburgh NYRate: $20.39/hrM-F 9-5:30 w/flexibility for 2 evenings/weekSeeking a Transitional Care Coordinator with lived experience with a mental health challenges and is willing and able to share that experience. The Transitional Care Coordinator will work with young adults ages 18-25.Job Description:In this role you will:Attend SPOA MeetingsShare your lived experiences to support the young adult in the programProvide services in the form of direct support information sharing and skill building.Coach educate support encourage young adult to use their voice and make informed choices.Increase young adult self-empowerment and advocacy skills.Mediate facilitate or bridge between young adult and agencies.Assist young adult in:Identifying natural supportsConnecting to informal supportsDiscovering their own strengthsEnsuring their plan is customized and individualizedDefining what success looks like the young adultResolving conflictsIntegrating into community school and familyAchieving successful independent living options Obtaining employmentTransitioning into adulthoodNavigating the public human services systemQualifications:The ideal candidate will have:Have a working knowledge of the MH education social employment and community services Must be able to relate to issues faces by young adults while supporting and providing a positive attitudeDemonstrated direct experience with their own mental health challengesAbility to work with a variety of agencies/organizations/schoolsMust be organized and able to prioritizeHave good organization writing and telephone skillsWillingness to participate in ongoing trainingsReport to the Senior Family PartnerNYS Certified Peer Specialist or be able to obtain certificationMust have dependable transportationRequired Experience:IC Key Skills Senior Care,Hoyer Lift,Medical office experience,Dementia Care,Home Care,Nursing,Alzheimers Care,Administrative Experience,Meal Preparation,Medication Administration,Memory Care,Tube Feeding Employment Type : Full-Time Experience: years Vacancy: 1
