Travels daily to provide community-based services to and on behalf of youth and families in compliance with organizational contract and regulatory requirements. Identify resources and services in the community that may not be available through the internal service continuum. Participate in the development of the youth treatment/service plans with the client caregiver(s) referring agency caseworker significant family member(s) and natural support. Regularly review the goals progress and activities of the assigned youth. Actively and directly engage with the youth to meet service needs and goals/objectives of the treatment/service plan. Interact with the youth to develop a trusting supportive relationship while assisting the youth in achieving identified goals. Provides safety assessment crisis response and behavioral stabilization services as required to assure safety and stability for youth served. Complete all relevant training on a timely basis as required by state contract and/or licensing agency standards. Complete all required documentation in a timely manner. Perform other duties as requested.Minimum Qualifications 21 years of age or older.GED or High School Diploma required. Associates Degree in social services with experience in child welfare is preferred. 2 years experience working directly with youth and families is preferred. A willingness to work flexible and non-traditional hours with afternoon and evening availability. Works well independently and as a team member. Multi-task efficiently and be flexible in all situations.
