Job DescriptionJob DescriptionGENERAL SUMMARY:Provide individual tracking and monitoring services through the Global Positioning System (GPS) to youth and families through a multi-agency community based collaborative service delivery system. Tracking of clients occurs throughout Milwaukee County at their homes, schools, jobs or community etc. Monitoring services occur 24/7 for all required and assigned youth to the program. Cross trained/dual role of Community Monitor and Youth Advocate to support both Level II and Intensive Monitoring Programs.ESSENTIAL FUNCTIONS:Complete daily phone check-ins and document in appropriate system(s) per contract requirements.Develop and maintain helping relationships with youth in program to encourage individual accountability.Complete curfew checks.Commit to work with any youth and family that is referred for the duration of their involvement in the program; acknowledge and emphasize the youth and family’s strengths, culture, and community.Monitor youth via phone, GPS monitoring or through collateral contacts (other professionals, etc.). Monitor will need to be able to respond to client locations to address any concerns with 2nd and 3rd shift needs.Enter data promptly in Agency and GPS tracking databases.Coordinate and complete assigned tracking/documentation per program expectations and requirementsReview case files to become familiar with youth history.Maintain all required trainings and certifications.Adhere to the mandatory tracking hours as designated by program guidelines for all assigned cases.Sustain timely responses to all inquiries by youth, families, Service Providers and Milwaukee County Youth & Family Services.Support the family decision-making process to empower the youth and the family to avoid further law violations or continued involvement in the Juvenile Justice System.Present required reports to contracting entities including court officials.Provide youth with transportation, as needed, to ensure that their needs are met.Prepare and complete all required documentation in an accurate and timely manner.Adheres to St. Charles Youth & Family Services Policies and Procedures.Supports the goals of the agency mission/vision.Other duties as assigned.Page BreakMINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:High School Diploma or equivalent; Associate’s degree in Human Services, Criminal Justice, or related field preferred.One year of experience with programs that serve a delinquent or at-risk youth population.Must have valid Wisconsin driver’s license, liability insurance, and meet company’s driving policy and expectations.Knowledge base of adolescent and family issues and dynamics.Familiarity with database systems with ability to enter data frequently and timely.Knowledge of Milwaukee County (Department of Human Services, Children’s Court, Milwaukee Public Schools, and Milwaukee community based youth serving agencies.)Knowledge of human growth and development theories.Ability to prepare formal reports as requestedAbility to present case in court if needed.Ability to document all interactions and activities in a timely manner.Ability to display cultural competence and sensitivity to the populations we service by responding respectfully and effectively to people of all cultures, socioeconomic characteristics, languages, classes, races, ethnic backgrounds, religions, and other diversity factors.Ability to accurately perform record keeping and documentation entries.Excellent verbal and written communication skills.Ability to display cultural competence and sensitivity to the populations we service by responding respectfully and effectively to people of all cultures, socioeconomic characteristics, languages, classes, races, ethnic backgrounds, religions, and other diversity factors.Ability to provide intervention programming and other programming as designed in collaboration with CYFS.·WORKING CONDITIONS:Indoor/outdoor requirementsEvenings and weekend requirementsIn accordance with the Americans with Disabilities Act, the above is intended to summarize the essential functions of and requirements for the performance of this job.
